If the reason is another case scheduled on the same date in another court, the motion shall include the name of the court, the case caption, the date and time of the conflicting case, and the date that the conflicting case was assigned for trial; (b) The time and date of the current assignment; (c) new date obtained from the Assignment Commissioner in the event the Court grants the motion for continuance.
